Early Intervention Process: Referral, Service Coordinator, Evaluation, and Services

Referral:

A parent may make an Early Intervention referral directly through their county by calling 311 for NYC and 914-813- 5094 for Westchester.

Service Coordinator:

Upon the approval of the referral, the agency will assign you an Initial Service Coordinator. This individual will be your direct contact for all EI related matters. They will also schedule an initial home visit to help further assess your needs and requirements.

Evaluation:

Depending upon your concerns with the child’s developmental growth, we will assign a state-approved professional to conduct an evaluation to assess the areas of concerns.

Services:

If your child is found eligible for services, we will collaborate with you to ensure that a licensed therapist is able to provide those services for your child. Services can be provided at home or community locations.
